Far harbor or the other Dlc is not required for 100% prior to their release. In other words the real 100% trophy doesn't include far harbor. Now that the dlc is added a previous platinum 100% went to 80% I think.So yes. The original platinum main game you need to do BOS, INS, and railroad endings to get 100%.
If you have the dlc and the added achievements the only requirement for your save is lvl15+ to get the additional 15 or so achievements. That save doesn't need to be an end game save. Hope that helps.
